Financial Aid Forms
Before you submit anything, make sure you're only filling out the form that the Financial Aid Office asked for or that appears on your Financial Aid Checklist.
Once you know which form you need, click the correct link below, follow the instructions on the form, and complete the upload steps at the end.
1) Dependency Override – Unusual Circumstances
If you have a serious reason why you can’t contact your parents, please fill out the Dependency Override – Unusual Circumstances form. Follow all instructions on the form.
If your parents don’t want to give their information—but you can contact them—do not use this form. Instead:

Fill out the student sections of the FAFSA,
-
Skip the questions about your parents,
-
You’ll only be considered for a limited Direct Unsubsidized Loan, and
-
Your Student Aid Index (SAI) won’t be calculated.
Important:
-
Only a Financial Aid Administrator (FAA) can decide if you must give parent info.

They may ask for more documents.
-
After reviewing your case, the FAA will decide if you can be considered an independent student.
2) Parent Non-Filing Statement Form
Use this form if your parent(s) did not file and were not required to file a 2023 federal tax return.
What to do:
-
Fill out the whole form,
-
Attach copies of any W-2s or other proof of income,
-
Follow the upload instructions at the end.
3) Professional Judgment Request Form
If your family has had a big change in finances, or if your dependency status needs review, you can ask for a Professional Judgment.
The Financial Aid Office will use this form to:

Look at your special situation,
-
Possibly adjust your FAFSA details, and/or
-
Review your dependency status.
Changes are made based on federal education laws and Department of Education guidelines.
4) Unaccompanied Homeless Youth Verification
Use this form if:
-
You said on your FAFSA that you’re homeless and not living with a parent, or
-
You support yourself and are at risk of being homeless.
Even if you didn’t check these boxes on your FAFSA, but you think you qualify based on the form’s definitions, you can still fill it out.
The Financial Aid Office may contact you or the people listed on your form for more details.
Upload Instructions
Follow these steps after opening the form:

Fill out all sections

Save with your changes by using the top right icon (don’t right-click)
You will see the below option when you select the download/save button in the top right hand corner. -
Save with changes

Save the file on your personal computer
-
Name the file using this format: FIRST_LAST_FAFSA_VERIFICATION.pdf
